FDC Assault Advisories | February 23 – March 1, 2021

This is a report of assault advisories that occurred during the specified timeframe. An assault advisory is issued by the Department of Corrections whenever a correctional officer or probation officer is attacked in the line of duty. All assaults are investigated, and disciplinary action is imposed in accordance with policy.

Santa Rosa Correctional Institution: On February 23, Inmate Rafael Rosario (L33284) assaulted an officer by kicking them in the leg. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Columbia Annex: On February 23, Inmate Mauricio Obando (M22358) assaulted two officers by striking them in the elbow and head. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Florida Women’s Reception Center: On February 23, Inmate Mayola Taylor (154415) assaulted an officer by striking them. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Santa Rosa Annex: On February 24, Inmate Jeremy Ward (149262) assaulted an officer by striking them in the face. Staff responded appropriately, and the inmate was subdued.

Santa Rosa Correctional Institution: On February 25, Inmate Jean Luxema (K84032) assaulted an officer by striking them with his elbow. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Santa Rosa Annex: On February 25, Inmate Jeremy Ward (149262) assaulted an officer by throwing his food tray and striking them in the leg. Staff responded appropriately, and the inmate was subdued.

Cross City Correctional Institution: On February 27, Inmate Harry Taylor (L98496) assaulted an officer by headbutting them. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Okaloosa Correctional Institution: On February 27, Inmate Brian Hughes (J57235) assaulted an officer by striking them on the leg. Staff responded appropriately, and the inmate was subdued.
